Feedback from Panthers supporters will help guide the club's fan engagement plans and partner offers for next season.
Members and fans are invited to take part in the annual survey, which takes approximately 10 minutes to complete.
The club says responses will help it understand what matters most to its supporters.
Submit the survey by Tuesday 27 October to enter the prize draw.
