Queensland in hot water with the NRL after a possible breach was uncovered during State of Origin 2, with the league asking for a 'please explain' over the curious scenes involving Reuben Cotter.

The NRL has reportedly written to the Queensland Rugby League for an explanation as to why Cotter was taken from the field for a head injury assessment (HIA) during State of Origin 2.

Cotter was upended in a tackle from Liam Martin in the 35th minute of the game at the MCG, with Martin being penalized and put on report.

Despite Cotter landing on his arm and not his head, the NSW forward was not sent to the sin-bin.

However, the Queensland forward was immediately taken from the field for an HIA, allowing the Maroons a free interchange due to the foul play.

According to the Sydney Morning Herald, the NRL is seeking answers from Queensland about why Cotter was sent for an HIA without hitting his head.

There is speculation that the Maroons may have been planning to take Cotter off the field regardless and seized the opportunity for a free interchange when Martin was put on report.

Cotter had previously clashed heads with Harry Grant, resulting in a nasty gash on Grant's forehead.

Despite this, Grant was not removed for an HIA, leading to doubts about whether the Maroons genuinely believed Cotter needed one after their collision.
